Failed IVF cycle? Here’s how you can increase your success rate!

IVF is an advanced fertility treatment which has a worldwide success rate between 30–70%. Although it is a well-established process, each step is complex and carries chances of failure. Several patients undergo 2–3 cycles before achieving success. Dr. Ramya Ramalingam, Chief Fertility Specialist, Jeevan Mithra Fertility and Women Care Centre, Arumbakkam, Chennai, shares some guidelines to improve chances of success.

APPROACH IVF TREATMENT AT THE RIGHT AGE

The woman’s age is especially important for IVF success. As every woman is different, the biological clock must be assessed with tests like Ovarian Reserve Assessment, to see if she can wait and try naturally or proceed to IVF.

CHOOSE THE RIGHT FERTILITY CENTRE FOR A FAVOURABLE OUTCOME

Considering the numerous fertility centres with various claims, choosing the right centre can be confusing. In addition to checking the centre’s success rates and team of specialists, also consider their live birth rate and success rate in critical cases, which reflect the embryology lab’s success. Also enquire on surplus embryo freezing facilities and seek first hand feedback and reviews.

ENSURE COMPLETE PRE-IVF EVALUATION

Women must undergo a general check-up, appropriate blood tests and vaginal and breast examinations to detect underlying conditions which may impact IVF success. A properly done trans vaginal ultrasound scan can reveal critical information to improve outcome. Fibroids, hydrosalpinx, endometrial polyps and low antral follicular count must be identified and treated before proceeding further.

Men must undergo semen analysis. The couple must maintain sugar control and ideal weight. Medication for pre-existing conditions must be changed to pregnancy-safe medicines under specialist guidance.

DECIDE ON PROPER TREATMENT PROTOCOL

IVF treatment offers various injection protocols. The best suited one must be decided ahead. Several techniques are also available to evaluate sperm and finalise on the right treatment. Further, the fertilisation procedure after egg collection must be planned beforehand. It can involve IVF, ICSI or other advanced fertility treatments like PICSI, IMSI and Microfluidic ICSI.

PROCEEDING WITH TREATMENT

There must be no compromise on the choice of hormone injection as it directly impacts the quality of egg, therefore resulting in superior grade embryo and IVF success rate. Recombinant injections are recommended. 10–15 eggs are ideal, so some can be used for the 1st cycle and others can be frozen for future pregnancy. Proper follicular and hormonal monitoring is critical to ensure adequate number and quality of eggs.

TIMING OF EGG COLLECTION

Egg collection is ideally planned 35–36 hours after trigger injections, failing which there is a chance for follicular rupture and low or no egg production. Once the egg collection is done, fertilisation is done according to the predetermined plan in the IVF laboratory.

Blastocysts which are embryos developed 5 days post fertilization have better implantation rate compared to day 3 embryos. Blastocyst transfer is especially recommended for those with previous IVF failures. Embryo growth and grading is important for IVF success and are determined by lab conditions.

EMBRYO TRANSFER

The number of embryos transferred is decided by the fertility specialist, while considering the woman’s age and embryo quality. Luteal support injections can be substituted with oral and vaginal medicines with the same effect, to make the process painless.

The woman must sleep well, avoid stress and protect herself from infection. She must eat freshly prepared, easily digestible food, without much oil and spice and follow medication as prescribed by the doctor.

TAILOR-MADE INDIVIDUALISED IVF PROTOCOL

Jeevan Mithra Fertility offers advanced ART techniques, for couples to achieve biological parenthood using their own gametes even at an advanced age. The centre boasts of a high success rate even in complicated cases with multiple failures.

In case of failure, couples should not lose hope and instead seek assistance from an experienced fertility specialist for troubleshooting and undergo further advanced tests and procedures, to improve success in repeat cycles. Jeevan Mithra Fertility has online consultation services for couples battling IVF failure.

What next after IVF failure?

Today, numerous couples are battling infertility issues, and several IVF centres have mushroomed across the country. Although IVF is regarded as the gold standard treatment, it offers only around a 30–70% success rate depending on the age of the couple, the problem, the woman’s egg reserve, the treating doctor, and the centre.

IS THERE ANY HOPE FOR PARENTHOOD?

Dr. Ramya Ramalingam, Chief Fertility Specialist, Jeevan Mithra Fertility and Women Care Centre, Arumbakkam, Chennai, says, “IVF failure only means that the particular cycle has failed, not that the couple can never conceive. Don’t let disappointment hold you back; there is still opportunity for parenthood if the right path is chosen.”

THE JEEVAN MITHRA ADVANTAGE

Over the years, Jeevan Mithra Fertility and Women Care Centre has helped over 10,000 national and international couples achieve their dream of parenthood. The clinic is said to specialize in helping patients suffering for long periods with infertility and those with multiple treatment failures. Jeevan Mithra adopts a comprehensive approach to offer effective solutions.

  • The experienced IVF specialists troubleshoot the exact cause of the problem.
  • The IVF failure patients are given proper counselling to ensure a positive state of mind. This helps some conceive naturally or with simple medicines and treatment protocol.
  • Individualized treatment protocols are suggested depending on the patient profile and problem. For hormonal issues, appropriate injections are given to improve egg quality. For sperm related concerns, correct sperm selection with microfluidic ICSI or a magnetic cell sorter is critical to enhance embryo quality and improve success.
  • The strong clinical team, well-experienced embryologist, and lab with optimal embryo growth rate play a key role.
  • Modern freezing protocols and infrastructure for freezing and storing embryos increase cumulative success, while being cost-effective. Jeevan Mithra Fertility Centre helps with own egg with natural cycle protocol, special stimulation protocol, low AMH and low egg reserve.
  • Low-cost protocol available for those with financial limitations.
  • All IVF protocols, including blastocyst culture, laser hatching, ERA, and PGS, are available under one roof. However, not all treatments are recommended for everyone. Precise treatment protocols and an individualized case approach increase the success rate in patients with IVF failure.

The biological clock is ticking. Couples planning parenthood need to be aware!

Ovarian aging is inevitable and plays a key role in enabling pregnancy. A woman’s biological clock refers to her fertility potential or ovarian reserve (the total number of eggs in the ovaries). A woman’s reproductive potential declines with age, especially after age 35, as there is a drastic reduction in the number and quality of eggs.

Ovarian aging is not uniform for everyone. Knowing one’s biological clock is critical as it varies significantly from individual to individual. For some, eggs might be aged even at a younger age, due to lifestyle, genes, and environmental factors. Early depletion of egg reserve is also becoming more common now.

BEING FOREWARNED IS BEING FOREARMED

Many couples seek fertility treatments to become parents as they find difficulty in conceiving naturally after a certain age. Couples looking forward to having a child need to have their problems assessed and seek appropriate treatment at the right time. When fertility treatments are started in the mid-30s, there isn’t much time to get pregnant in a relaxed and positive manner. The delay can lead to failure of treatment, which in turn will increase stress and anxiety. Hence, couples need to prioritize childbearing rather than postpone it.

Dr. Ramya Ramalingam, Chief Fertility Specialist, Jeevan Mithra Fertility and Women Care Centre, Arumbakkam, Chennai, says, “As every woman’s biological clock is different, Ovarian Reserve Assessment is essential to see if she can wait and try naturally or proceed to IVF. The assessment involves a simple scan and blood test to reveal the number of eggs in the ovary at a particular point of time.”

Since there are so many fertility treatments available, couples can feel confused or overwhelmed about the way forward. Dr. Ramya Ramalingam explains, “An experienced fertility expert can recommend necessary investigations and suggest the right treatment according to the results, for increased success. Jeevan Mithra Fertility Centre believes that IVF is not the only solution for infertile couples. We recommend various simpler treatment options to help the couple conceive. For instance, we offer a low-cost IVF protocol in cases where eggs are good, but there may be tube problems or sperm problems. This financially viable procedure employs mild injections and minimal stimulation, with a success rate.

BUSTING COMMON MYTHS RELATED TO IVF TREATMENT

Despite the high success rates, many people have wrong preconceived notions, making them skeptical about IVF. Dr. Ramya Ramalingam clarifies these doubts.

Does IVF interfere with the natural process?

Never! During IVF, specialists assist the natural process to ensure fertilization and development of embryos and transfer it back into the uterus. Implantation which is a critical step to achieve pregnancy, is left to nature.

Does a failed IVF cycle mean the end of parenthood dreams?

No, there is still hope. Many recent technologies like PGS, non-invasive PGS, ERA, laparoscopy, hysteroscopy, corrective surgery, laser hatching, and special selection methods for ICSI are available to increase the implantation rate and success rates in those with previous IVF implantation failure.

Will the embryo get mixed up at the fertility centre?

Jeevan Mithra has an intelligent IVF system, where a complete ID check is undertaken at each and every stage. An alarm system is also in place to ensure no errors. Couples can proceed with the IVF procedure with confidence.

Jeevan Mithra Fertility Centre offers several advanced fertility treatments to help couples facing recurrent implantation failure. Since age is a critical factor for all fertility treatments, don’t delay seeking appropriate advice and treatment. Even the most complicated cases can meet success with treatment at the right place at the right time.
